Employee Benefits Dependent Verification - Extended Deadline

You MUST Verify Your Benefits Dependents by Thursday, Sept. 15 by 11:59pm,
if you receive benefits from RHA AND have dependents on your plan(s).

IF you miss this deadline:
  • Your dependents will be removed from that benefit plan effective 10/9/16
  • Your dependents will not be eligible to be added back until this Fall's Benefits Open Enrollment (with benefits starting back up on 1/1/2017)
  • You may be at risk for tax implications/fines if you or your dependents are not covered under an insurance plan.

What Employees Need To Do?

  1. List eligible dependents on your plan
    (RHA coverage options include medical, dental, and/or vision insurance to our free basic life insurance offered to spouses and children)
  2. Gather copies of eligible documentation
  3. Send to the RHA Benefits Service Center by Thursday, September 15 before 11:59pm
  4. Employees can check the status of your dependent verification by calling the RHA Benefit Service Center 1-844-836-0979

Why Do Employees Need To Verify Dependents?
  • The reason for this verification is to meet audit requirements of RHA’s health plan.
  • New hires will complete the dependent verification when they initially sign up for benefits.

Acceptable Types of Dependent Documentation 

NOTE:  RHA will not see or receive any of this documentation.  This information is sent to RHA's Benefits Service Center, which is a 3rd party RHA contracts with to manage benefits enrollment and support.


Eligible dependents include:

Spouse — Your legal spouse.

 Children — Your unmarried children up through 26, including your biological or step children, as well as children you have adopted or who have been placed with you for adoption. You may also cover a child for whom you have legal custody or guardianship and disabled adult children over age 26.

Option 2 — if Option 1 is unavailable, please provide all documents noted in the chart to satisfy Option 2.  Documents must be unexpired and valid.   A printout of an online marriage certificate may also be an acceptable document.
